 Living Will

Power of Attorney

The Last Will and Testament is a written directive of how you wish your estate to be handled. The last will and testament form appoints an executor to handle your affairs and distributes your assets to the beneficiaries named by you in the will.

The Living Will communicates your wishes regarding treatment when those wishes can no longer be communicated by you. The living will form designates an individual to make health decisions for you at a time you are unable to do so.

Power of attorney forms including general power of attorney, durable power of attorney, medical power of attorney, health care power of attorney, limited power of attorney, real estate power of attorney, enduring power of attorney, financial power of attorney, special power of attorney and power of attorney revocation. The power of attorney form allows another person to act on your behalf on matters you specify.

Revocable Trust

Irrevocable Trust

The Revocable Living Trust is a legal arrangement used in estate planning that provides for the management and distribution of your property.

The revocable living trust form, revocable and amendable by you during your lifetime, is useful in providing children with a fund for education or other specific planning purposes.

The purpose of the creation of the trust is to provide for the convenient administration of certain assets without the necessity of court supervision in the event of the trustor incapacity or death.

Other provisions within the document include: trust assets, the abstract of the trust, and suggested trust registration.
